A practical guide to understanding which health-insurance arrangement you need to check in Japan.

Start with the system that may apply to you

Japan has more than one health-insurance arrangement. The practical route can depend on employment, household circumstances, residence and the organisation administering your coverage. Check the system that applies to your current situation instead of treating all coverage as one system.

National Health Insurance

Municipalities administer National Health Insurance for people to whom it applies. Local procedures and contribution amounts can vary, so confirm them with your own municipality.

Employees’ Health Insurance

For covered employment, the employer and insurer administer the practical procedure. Japan Pension Service explains that Employees’ Health Insurance is linked with Employees’ Pension Insurance at covered workplaces.

Dependent coverage

Dependent arrangements are system-specific. Do not assume that a household relationship alone confirms eligibility; check with the employer, insurer or other responsible administrator.

Changes in work or address

Starting or leaving work, changing employer and moving can change what you need to verify. Check the relevant employer, insurer and municipality before relying on old information.
